Pain in Lower Right Abdomen – causes and symptoms

We can divide abdomen with 4 imaginary quadrant.
Following organs are located In the Right lower quadrant:
appendix, the upper portion of the colon, and the right ovary and the Fallopian tube in women.
Generally, abdominal pain is common and most cases are mild, without any serious underlying cause.
In Most cases abdominal pain is caused by indigestion, gas, constipation, muscle spasm, muscle strain or gastritis.
But when pain is severe and located, specifically on right lower quadrant of abdomen, then most common cause is appendicitis.
Appendicitis is inflammation of the appendix. Appendix is a part of intestine.
Treatment of appendicitis surgical removal of appendix and it is called appendectomy.
Appendectomy is the most common surgical operation of abdomen.
Appendicitis has its highest incidence between the ages of 10 and 30 years.
Untreated, appendicitis can lead to perforation of the appendix, which typically occurs within 24 to 36 hours of the onset of symptoms. Perforation of the appendix can cause intra-abdominal infection, sepsis, intraperitoneal abscesses, and rarely death.
In order to avoid the sequelae of perforated appendicitis, a low percentage of “negative” appendectomies is generally accepted from a surgical standpoint.
If we suspect appendicitis surgical consultation should be provided, without any delay.
And we can suspect on appendicitis with the following symptoms:
abdominal pain migrating to the right lower quadrant (RLQ),
Pain from appendicitis may begin as dull pain around the navel. After several hours, pain will usually transition towards the right lower quadrant, where it becomes localized.
anorexia, fever, and nausea/vomiting. On examination, RLQ tenderness (hard), along with “classical” signs of peritoneal irritation (e.g., rebound tenderness, guarding, rigidity, referred pain).

Rebound tenderness, also known as Blumberg’s sign, refers to the presence of pain when pressure is removed from the abdomen, rather than when applied.
Abdominal guarding is the tensing of the abdominal wall muscles to guard inflamed organs within the abdomen from the pain of pressure upon them. The tensing is detected when the abdominal wall is pressed.
although many patents can have atypical presentation, so appendicitis can possible partial or without abomination symptoms.
Imaging tests, such as ultrasound (US), computed tomography (CT) with and without contrast, and magnetic resonance imaging (MRI), are also used extensively for the diagnosis of appendicitis.

Among Women, other serious causes of lower right abdominal pain can be ovarian cyst rupture, or adnexal torsion.
Other causes can be: Kidney stones, kidney infection, hernia, rarely, inflammatory bowel disease (IBD),
irritable bowel syndrome (IBS).

Appendicitis is the most common cause of right lower abdominal pain requiring surgery.
The pain from appendicitis typically starts near the navel and shifts to the right lower abdomen, becoming sharper over time.
Increased pain with coughing, movement, or jarring activities is a key indicator of appendicitis.
Young people and teens are more prone to appendicitis, often due to a blockage by stool.
Over 80% of appendicitis cases necessitate surgical intervention to prevent life-threatening complications like peritonitis.
Diagnostic tests for appendicitis include ultrasound and CT scans, with Rovsing’s sign and rebound tenderness being clinical indicators.
Kidney stones are another major cause, characterized by severe pain starting from the back or side and potentially moving towards the groin.
The pain from kidney stones is often described as the worst imaginable and is significantly different from that of appendicitis.
Ovarian cysts, fluid-filled sacs on or in the ovaries, can cause a dull ache or sharp pain, especially upon rupture.
Gastroenteritis, or stomach flu, leads to crampy, diffuse pain and is accompanied by symptoms like bloating, diarrhea, and vomiting.
Irritable Bowel Syndrome (IBS) typically causes chronic discomfort and pain, not specifically in the lower right abdomen but possible.
Inflammatory Bowel Disease (IBD) includes conditions like Crohn’s disease, which often causes pain in the lower right abdomen.
Anti-inflammatory medications are commonly used for IBD treatment, with surgery as an option in severe cases.
Additional causes of lower right abdominal pain include endometriosis, pelvic floor dysfunction, ectopic pregnancy, diverticulitis, and intestinal herniation.
The majority of conditions causing lower right abdominal pain require medical diagnosis and treatment, highlighting the importance of professional evaluation.

Common Causes of Left Side Chest Pain

In this video, we will explore some of the most common reasons why people experience pain in the left side of their chest.

One of the most common causes of left side chest pain are:
Esophageal Rupture
An esophageal rupture is a medical emergency that may cause non-cardiac chest pain. This condition occurs when the tube that connects the mouth to the stomach tears, allowing substances from the mouth to leak into the chest cavity and the lungs.

Sometimes, a person may experience this type of injury after excessive vomiting or experiencing physical trauma around the esophagus.
Myocarditis
Myocarditis is an inflammation of the heart tissue, specifically the myocardium, the middle layer of the heart wall. Doctors often cannot identify the cause, but when they can, it is typically due to a viral infection.

Chest pain can be a sign that your heart muscle is inflamed. Other signs include:

Fatigue
Shortness of breath
Rapid or irregular heartbeat (arrhythmia)

Myocarditis can weaken your heart or cause permanent damage to the cardiac muscle. In some cases, a person can experience cardiac arrest or even death.

In this case, we think of 2 things, that is any problem in the colon as it is there in the right side of the body. Secondly, any problem in the appendix. Third is any small stones in the kidney tubes or the ureter. These are the three common reasons. When there is pain in the kidney tubes, pain also goes to the genitals and have an associated genital pain. To summarize, any problem below the umbilicus, you should think of intestine problem, intestine infection, appendicitis, kidney related issues, or ureter problem. In case of females, we also look at any problem in the ovary, uterus and the tubes of the uterus, or any infections in the bladder.

Night leg cramps – Leg Cramps at Night: Causes, Pain Relief & Prevention

Up to 60 percent of adults have had nocturnal leg cramps.
Nocturnal leg cramps are involuntary muscle contractions that occur in the calves, or soles of the feet, during the night.
Leg cramps are painful and Generally temporary and non-damaging, lasting several seconds to minutes. It can cause significant pain and a paralysis-like immobility.
Exercise research suggests that muscle fatigue is a primary cause of leg cramps.
Night cramps can be normal at late stage of pregnancy.
In other cases it is associated with physical activity, exercise or overexertion, age (common in older adults), lack of electrolytes, such as magnesium, potassium and sodium.
Prolonged sitting or laying reduced blood flow. Muscle fatigue can be considered one of the most important risk factor.
Rarely muscle cramps may be a sign of a motor neuron disorder like lateral amyotrophic sclerosis and multiple sclerosis, but such cases are rare.
Some other Medical conditions can be associated with leg cramps are cardiovascular disease, hemodialysis, cirrhosis, pregnancy, and lumbar canal stenosis.
Other conditions that can mimic night cramps include: restless legs syndrome, claudication, myositis, and peripheral neuropathy. All of them can be differentiated through careful history and physical examination.
Management.
Gentle stretching and massage, putting some pressure on the affected leg by walking or standing, or taking a warm bath or shower may help to end the cramp. If the cramp is in the calf muscle, dorsiflexing the foot will stretch the muscle and provide almost immediate relief.
There is limited evidence supporting the use of magnesium, calcium channel blockers, carisoprodol, and vitamin B12.
Based on this first large, representative study, Nocturnal Leg Cramps occurring 5x per month are reported by 6% of the adult US population. Sleep disturbance symptoms and health conditions are associated with higher frequency of Nocturnal Leg Cramps, suggesting that Nocturnal Leg Cramps is a marker, and possibly contributor, to poor sleep and general health.
Prevention
Adequate conditioning, stretching, mental preparation, hydration, and electrolyte balance are likely helpful in preventing muscle cramps.

Classic coronary pain–or angina–involves a substernal pressure that commonly begins with exertion and is relieved by rest. However, some patients experience angina in the absence of physical exertion or emotional stress, and not all chest pain that begins after exertion is angina. Atypical chest pain must be differentiated from other types of chest pain, including chest wall pain, pleurisy, gallbladder pain, hiatal hernia, and chest pain associated with anxiety disorders. Careful examination of the chest wall is essential, and abnormal heart sounds can tell you a great deal. Further testing is individualized. An exercise ECG is important in identifying the presence of ischemic heart disease and the amount of myocardium at risk. If the ECG is abnormal at rest, the patient should undergo a thallium stress test or exercise echocardiography. A coronary angiogram is indicated if the exercise test or an ECG during pain show that a lot of live heart muscle is at risk.

Unilateral leg pain can happen in some conditions like diabetes where the nerves are affected. It can also be seen in conditions where the nerves are directly affected, nerves which move down from the spinal cord to the lungs. So any compression or irritation to the nerve at any point can cause a pain in the leg on that particular side. In children it is very common to see the leg pain because of the growth of the bones. It can be unilateral or bilateral. In menopause women do complain of unilateral leg pain. Any condition affecting the artery or the vein where the circulation is affected then that can cause leg pain. If the veins are not able to take the blood across it because of a clot or because it is dilated too much as in varicose veins then that also can cause unilateral leg pains.
